The Evaluations Module gives you the ability to create custom surveys, feedback forms and questionnaires, and to report back on your findings.
You can link responses to profiles or keep them anonymous.
You can also create templates that allow you to collect both qualitative and quantitative data and report more easily. With no limits to the number of templates, you can create as many as you need for your different data collection requirements.
This module pairs well with the Data Direct module, that allows your participants to fill in their responses online – saving time and paper.

Already a Lamplight user?
To enable the module, go to system admin (you must be a system administrator) > change modules and tick the Evaluations Module box to turn it on in your system.
